As part of using BetterDoc services, personal data such as name, address, date of birth, phone number, and health insurance information, as well as special categories of personal data related to the health issue, will be processed.

Therefore, we ask you to provide data privacy consent with the following content as part of your request.

I consent to BetterDoc GmbH, Kaiser-Wilhelm-Ring 30-32, 50672 Cologne, as the data controller, processing the following personal data, including special categories of personal data (health data), for the purposes listed below:

Release from Medical Confidentiality for Appointment Scheduling & Second Opinion Assessments

If you authorize BetterDoc to schedule an appointment at a clinic/practice or to obtain a medical second opinion from a designated physician, you consent to BetterDoc being released from medical confidentiality for these purposes.

Revocation

You can revoke your consent at any time with future effect, without any formal requirements and without providing reasons (e.g., via email to datenschutz@betterdoc.org or by phone at 0800 / 444 6 999). The legality of the processing carried out based on your consent until the revocation remains unaffected. Upon revocation, your data covered by the consent will be deleted immediately, provided there are no legal or contractual retention periods. If consent is not granted or is revoked, we will not be able to process your request further.
